In order to make your stay at the University Hospital Magdeburg as pleasant as possible, we have compiled this checklist, which will help you to prepare for your stay at the University Hospital Magdeburg.
Important documents for your inpatient stay.
- Documents for your admission to our clinic
- Uniform national medication plan
- Referral certificate issued by your family doctor or specialist
- Identity card or passport
- Health insurance card or written cost assumption declaration from the private health insurance company
- Health passports, if available
- if applicable, proof of eligibility for aid (last aid decision)
- current x-rays, CT images and other preliminary findings
- Findings from previous hospital stays
- Documentation of the social ward with existing outpatient care
- vaccination card
- other health passports: cardiac/pacemaker ID card, Marcumar pass, allergy, X-ray, diabetic, prosthesis pass, etc.
- Doctor's letters
- Precautionary Powers/Patient Decree
Personal things for your stay
- Toiletries, hairdryer, nail scissors and file
- towels, washcloths
- pyjamas or nightgown, bathrobe
- tracksuit or comfortable clothes, stockings
- non-slip slippers
- Sneakers / comfortable, sturdy shoes
- Underwear and clothing for discharge from hospital
- personal aids (e.g. glasses, walking stick, hearing aid, support stockings, etc.)
- Things for personal needs (address and telephone list, reading, writing materials, some money)
